    Requesting an OASI card

    If you do not have an OASI card or if you have lost yours, you can request one by filling out the online form and sending it to the competent authority.

    Good to know

    If you have lost your card, your new card will have the same number as the previous one. The OASI card ceased to be compulsory in 2016. However, you can find your OASI number on your health insurance card.

    If you have not yet received your OASI card or if you have lost it, you can fill out the application form available on the OASI/DI website and return it to your employer.

    You will then be sent a (new) OASI card.

    If you have not yet received your OASI card or if you have lost it, you can fill out the application form available on the OASI/DI website and return it to your professional fund association or your cantonal compensation office to which you make your contributions:

    If you live abroad and do not work in Switzerland and have never received or have lost your OASI card, you should contact the Swiss Compensation Office, which can issue with you with one or provide a replacement.
